If you are a new patient/client who has already contacted us or a current patient/client, you may also request an appointment through your Patient Ally Patient Portal. If you are requesting a telehealth appointment, please type "Telehealth" into the "Add a note to your provider" field in Patient Ally. You may also review our privacy policy.

REQUEST AN APPOINTMENT

Thank you for requesting an appointment! We will get in touch with you within 48 hours. For new patients, we will email you a Patient Ally enrollment invitation link. Once you have enrolled with Patient Ally, we will send you intake documents through the Patient Portal. Please complete and submit the intake documents a minimum of 24 hours before your first appointment.
